• Print Three (3) Copies of the Request for Live Scan Form
• For a Personal Use Fingerprint Background Check: Record Review Live Scan Request Form

Request for Live Scan Form

Fill out your personal information on the Request for Live Scan Form.

• Required Fields: Name, Date of Birth, Address, Gender
• Recommended Fields: Social Security Number, Driver’s License Number, and Phone Number

Pay For Fees Online

Paying for the Live Scan service online is optional, benefits include:

• Faster processing on-site.
• More secure and all credit cards accepted.
• This is ideal for organizations and employers.

Live Scan Locations

Travel To Location

Drive to Live Scan location.

• Being one of the largest Live Scan service providers in California, most of our locations are within a ten minute drive from your home!
• If you cannot travel to one of our locations, you can also request a mobile live scan service.

Fingerprinting Service

A Trained and Certified Technician will scan your fingerprints using a Live Scan machine.

• The electronic fingerprinting session takes five to ten minutes and your prints are immediately sent via electronic transmission to
  the appropriate FBI-Certified channeling agencies for background check processing.
• If you have not paid online, the live scan technician will collect all the appropriate fees at the live scan site.
