FD-258 Fingerprint Cards & Fingerprint Clearance Card Support

Fingerprinting for Arizona Requirements

Certifix provides professional fingerprinting services for individuals who need fingerprints for Arizona licensing, employment, or Arizona Department of Public Safety (DPS) requirements.

Arizona commonly requires applicants to apply for a Fingerprint Clearance Card (FCC) through the Arizona Department of Public Safety. As part of that process, applicants must submit fingerprints on an official FD-258 fingerprint card.

Certifix captures your fingerprints digitally using Live Scan equipment and professionally prints them onto an official FD-258 fingerprint card. You will then submit the fingerprint card along with your application and required payment directly to Arizona DPS.

Please note: Certifix does not issue Fingerprint Clearance Cards. Clearance cards are issued by the Arizona Department of Public Safety.
