Certifix offers FD-258 fingerprint card printing at most locations. Hundreds of our sites are equipped with both Live Scan and ink capabilities to produce high-quality fingerprint cards. If we’re unable to capture your prints during your appointment, we’ll gladly help you reschedule at a time that works best for you.
You can also choose to have your fingerprints captured and printed on an FD-258 card at any participating location. Our professional staff will take your fingerprints using a Live Scan machine and print the card for you to take home.
We can also digitally submit your fingerprints to the FBI as a Departmental Order. Your fingerprint file will be securely transmitted to the FBI through our approved channel.

Use our nationwide directory to locate a Certifix fingerprinting site near you. Many of our locations offer FD-258 fingerprint card services using Live Scan capture.
Before visiting, contact the location to confirm they provide FD-258 card printing. Only select locations can print and hand you the card during your appointment; at most locations, the card will be mailed to you after your fingerprints are captured.
Visit the location, where a trained fingerprinting operator will capture your fingerprints using a Live Scan system. Your FD-258 fingerprint card will then be mailed to you, or provided on-site at participating locations.
For most customers, you won’t need to fill out the FD-258 fingerprint card yourself. When your fingerprints are captured using our Live Scan system, your information will be automatically printed onto the card—either at the location (select sites) or before the card is mailed to you.
However, here are the general guidelines that apply when information must be completed manually:

Yes. You can download and print a blank FD-258 card as a PDF. However, many agencies require fingerprints to be taken by an authorized provider.
Yes. You can have your fingerprints taken at a fingerprinting facility such as Certifix. Bring a valid government-issued photo ID and the applicable fee.
Traditional FD-258 cards can be completed with ink, but Certifix does not use ink. We capture fingerprints digitally using a Live Scan system and print them onto the FD-258 card.
FBI background checks search your entire lifetime criminal history. There is no 7-year limitation.

Yes. FD-258 cards must be submitted to the requesting agency unless you are using Certifix for an FBI Departmental Order, in which case we can submit electronically to the FBI for you.
The required fields on the FD-258 fingerprint card typically include your last, first, and middle name; any aliases (AKA); date of birth; place of birth (state and country); sex, race, height, weight, eye color, and hair color; your Social Security number if required; your residential address; and employer or agency information when applicable.
Your Certifix technician will complete the required fields for you when using Live Scan. If filling out manually, follow the instructions provided by the requesting agency.
Complete all required personal and demographic fields and ensure accurate information. Missing details may delay processing.
The ORI (Originating Agency Identifier) is a 9-character code that identifies the requesting agency. It must be provided by the agency requiring your fingerprints.
Do not submit an FD-258 card with your USCIS application. USCIS will schedule biometrics separately and will not accept fingerprint cards submitted by mail.
