Card Printing Instructions

For most customers, you won’t need to fill out the FD-258 fingerprint card yourself. When your fingerprints are captured using our Live Scan system, your information will be automatically printed onto the card—either at the location (select sites) or before the card is mailed to you.

However, here are the general guidelines that apply when information must be completed manually:

  • Use black or blue ink if any fields need to be handwritten.
  • Do not fold or bend the fingerprint card.
  • Ensure all information is accurate to avoid delays or rejection.
  • In the NAM – NAME field, list your Last Name, First Name, then Middle Name.
  • Your signature must match the name printed on the card.
  • Only residential addresses are accepted; PO Boxes are not allowed.
  • The date and official signature areas are reserved for official use.
  • Enter your employer’s name and address if applicable.
  • For AKA – ALIASES, list previous legal names or maiden names (not nicknames).

Certifix charges $30 to $40 for the FBI fingerprinting process at all of our locations, with an additional $10 for each extra FD-258 card. Your fingerprints will be captured digitally using our Live Scan system, and a printed FD-258 card will be provided or mailed to you for submission to any agency or department.
