Yes. Many Utah agencies and licensing boards accept FD-258 fingerprint cards when mailed as part of an application packet. For example, the Utah Division of Professional Licensing (DOPL) allows out-of-state applicants to include completed FD-258 cards with their licensing forms if they cannot be printed electronically at a vendor location.
Fingerprints are captured electronically using Live Scan equipment and printed onto an official FD-258 fingerprint card. In most cases, the completed card is mailed to you after your appointment so you can submit it with your application to the appropriate Utah agency or licensing board.
