How do I get a fingerprint card in Arizona?
Certifix provides fingerprint card services in Arizona by capturing your fingerprints and printing them onto an FD-258 fingerprint card. If you visit the Certifix Phoenix location, the fingerprint card is typically provided the same day. At all other locations, the completed fingerprint card is mailed to you after your fingerprinting session. You then submit the card with your application and payment to the Arizona Department of Public Safety.
