To apply for an IVP (Identity Verified Prints) Fingerprint Clearance Card, submit an application to Arizona DPS and include fingerprints on an FD-258 card. Certifix provides the fingerprint card after capturing your fingerprints. You then mail the card, application, and payment to DPS as instructed.
