How do I replace a lost Fingerprint Clearance Card in Arizona?
If your Fingerprint Clearance Card is lost, request a replacement directly through Arizona DPS. Replacement cards are issued by DPS and mailed to you. Certifix does not replace clearance cards but provides fingerprinting services for new applications when needed.
